Plus Reformer Access Upgrade
Request to Upgrade to Plus Reformer Access membership
This form is for current PARC members to request a membership upgrade to the new Plus Reformer Access membership during the pre-sale period 1 May - 26 May 2025. The PARC Reformer studio opens on Thursday 29 May 2025. Bookings for PARC Reformer classes will open for Plus Reformer Access members 74 hours prior to the class start time. By filling in this request form, you are giving permission for the Sales Team to process a membership upgrade using your current information in the Xplor Customer Portal. If you have any questions prior to requesting this membership upgrade, please call our Member Experience team first on 9781 8444.

Membership Upgrade Agreement

I am requesting to upgrade my current membership to a Plus Reformer Access fortnightly membership. I give permission for a Sales Team member to process a membership upgrade from my current membership to a Plus Reformer Access membership with an Activation date of 28 May 2025. The last active day of my current membership will be set to 25 May 2025. Any credit applicable from the cancellation of my current membership billing period will be credited against my first fortnightly membership payment to commence my Plus Reformer Access membership.

The new membership cost for the Plus Reformer Access membership will be equivalent to my current fortnightly membership rate plus an additional $19.90 per week (an additional $39.80 per fortnight). I will not be required to pay a joining fee to upgrade my membership.
3.

I understand that my first Plus Reformer Access membership payment will be deducted on 26 May 2025 equivalent to one fortnightly debit. The fortnightly payment schedule for my new membership will then commence from Thursday 12 June 2025 and payments deducted on this day and then fortnightly thereafter. I confirm that my membership profile in the Xplor Customer Portal has valid saved Financial Info and I give permission for a Sales Team member to select this saved Financial Info as the payment method for my Plus Reformer Access fortnightly membership.
